Logon Requests - Virginia Elite/Hospital Hub/VSTR

Located within this article are the forms required to be filled out to obtain access to any of the following systems:

• Virginia Elite (as well as the older VPHIB (version 2) system if needed))
• Virginia Hospital Hub
• Virginia Statewide Trauma Registry (VSTR)

Virginia Elite system access setup by OEMS is limited to EMS agency administrators that have the authority to manage their EMS agency's medical records. Individual EMS provider accounts are managed by each agency's VPHIB administrator. Individual EMS provider users’ accounts are not managed by OEMS staff.

To obtain access to VPHIB or VAv3, the following forms need to be filled out:

• 1 - Logon Request
• 2 - VPHIB EMS Admin Security Agreement
• 3 - VPHIB EMS Admin Delegated User Template (be sure to review the instructions included on the sample form - Form number 3 is just an example of what must be submitted.  It is not intended to be filled out)

Hospital Hub is a Virginia Office of EMS managed database where hospitals, primarily emergency departments, can retrieve EMS electronic medical records (EMR) for the patients that arrive by ambulance to their hospital.

To obtain access to Hospital Hub at a user level, the following forms need to be filled out:

• 1 – Logon Request
• 4 – Hospital Hub Security Agreement

To obtain access to Hospital Hub at an administrator level, the following forms need to be filled out:

• 1 – Logon Request
• 4 – Hospital Hub Security Agreement
• 5 – Data Use Agreement – Hospital Hub (note that page 4 of this agreement lists the typical individual this needs to be signed by and it should be signed by an individual with the authority to enter into a contract with the Commonwealth of Virginia)

*Note - If a facility has already established a Hospital Hub Administrator, then that individual and not OEMS will be responsible for setting up new accounts at the facility. If you are unsure, please contact support via email, and support personnel will verify how best to proceed.

VSTR user access is granted to hospital staff responsible for submitting the mandatory hospital trauma dataset.

To obtain access to VSTR, the following forms need to be filled out:

• 1 – Logon Request
• 6 – VSTR Security Agreement
